One FQHC’s Integrated Mobile Unit with Primary Care, Mental Health Care & Substance Use Treatment

clinic

PanCare, a Federally Qualified Health Center (FQHC) in Florida’s panhandle, serves 60,000 patients across nine rural counties through its Mobile Behavioral Health unit. Covering 4,000 square miles, the unit offers primary care, behavioral health counseling, medication management, Substance Use assessments, and Medication Assisted Treatment (MAT), allowing PanCare to bring essential services directly to patients. Equipped with telehealth capabilities, the unit ensures timely care even if a provider is unavailable on-site. An Outreach Case Manager provides screenings, education, and support for conditions like diabetes and smoking cessation. MAT services, including intake, diagnosis, and medication administration, are provided on-site, overcoming referral and transportation challenges. The program’s sustainability relies on grant funding and service billing.
