Strengthening & Sustaining Mobile Healthcare: Grant Program Application

Overview
This year, the Strengthening & Sustaining Mobile Healthcare Grant Program will focus on program monitoring and evaluation. This grant opportunity will allow members to develop a structured approach to collecting, analyzing, and reporting essential data to enhance program effectiveness while identifying areas for improvement.
Setting up our members for success is vital to the future of mobile healthcare. This grant program will help our members develop a strategy to demonstrate evidence of value to stakeholders, secure funding and support, and enable continuous adaptation to community needs.
Additionally, the plan can aid your program in optimizing resources, ensuring that limited resources are used efficiently to maximize health outcomes. When developing a grant proposal, include a comprehensive plan that shows how you will measure your performance.
Funding Details
Award up to $10,000 to ten Association members (in good standing) across the U.S. and Canada. The funds will be used to develop a structured approach to collecting, analyzing, and reporting essential data. Funds must be fully used by April 30, 2025. A short report must be submitted by May 30, 2025, to klieberman@mobilehealthca.org
Grant Award
Selected programs will be awarded up to $10,000 each. Allowable evaluation expenses may include:
• Personnel costs – including salaries for evaluation staff and consultants
• Research and planning expenses – such as purchasing evaluation tools and software
• Data collection and analysis costs
• Staff training expenses
• Mobile programs may also incur costs related to stakeholder engagement activities, and developing and validating instruments
Application Timeframe
Applications will be accepted beginning September 27, 2024, through October 25, 2024.
Eligibility Requirements
Applicants must have non-profit status or a fiscal sponsor affiliated with a nonprofit organization, or state or county agency. Mobile healthcare programs can provide any type of services, (i.e.: prevention, dental, pediatric, clinical, vision, general practice) but must serve marginalized or underrepresented populations in the U.S. or Canada. Eligible applicants must be:
• Mobile Healthcare Association member in good standing
• Program currently operating a mobile program
• Program in operation for at least one year as of 10/1/2024
• Non-profit organization with 501C (3) status or state or county agency
Grant Important Dates and Timeline
Application Start Date | September 27, 2024 – October 25, 2024. | |
Q&A Sessions | Mobile Healthcare Association will host a Q&A Session on October 2, 2024, at 2:30 AM ET. Click here to register. | |
Application Review and Award Notification | Applications will be reviewed between October 25, 2024 – November 15, 2024. | |
Grant Agreement | Upon receiving the award, grantees must sign an agreement within seven business days of receipt from the Association. | |
Invoice for Grant Award | The grant award will be released to the grantee by December 9, 2024. | |
Final Report Requirement | The grantee must complete the project within the agreed-upon timeframe. Within one month of completing the project, the grantee must provide a final report on how the funds were used. |

Submission Requirements
An expert panel will review, and rate proposals based on the criteria listed below:
a. Criteria
1.Project Description including mission, program objectives, and a detailed description of who your mobile program services.
2. Proposed Project: Details of your proposed plan to develop a monitoring and evaluation plan.
3. Project Narrative: A description of the project and timeline for implementation and anticipated outcomes.
b. Submission Requirements
a. Applicant must submit an online application detailing the following:
• Your programs mission, target population and services provided
• Your project’s purpose, objectives, and desired outcomes
• Provide a budget
• Provide a current IRS tax-exempt letter, W9, a logo file, and a photo of your clinic in service.
c. Completion of your project
• After completing your project, you will share a brief description of the successes and challenges or changes to your mission or goals.
• Mobile Healthcare Association may contact grantees to request status updates.
