This interactive workshop is designed for frontline health workers and clinicians working with individuals living with or at risk for diabetes in mobile clinic settings. Participants will explore practical strategies to enhance diabetes care and chronic illness management in underserved communities. Through hands-on practice and discussion, the workshop will cover how to integrate A1C testing into mobile workflows, identify common barriers to diagnosis and treatment, and use culturally appropriate educational tools to support health literacy. Special attention will be given to addressing food insecurity and providing tailored nutritional education, including the creation of healthy food boxes—to support long-term, sustainable lifestyle changes. Attendees will leave with actionable tools to deliver more equitable, effective, and patient-centered care.  

Learning Objectives: Demonstrate how to effectively integrate A1C testing into point-of-care services within mobile clinics serving underserved populations, including setup, workflow, and follow-up strategies, ordering supplies. Lead with curiosity and empathy when assessing patient barriers to adherence. Identify common patient barriers to the diagnosis and management of chronic illnesses, using role play, case studies, and motivational interviewing techniques, and apply culturally responsive communication to support improved engagement and care adherence. Consider the relationship between adherence and patient learning materials and adapt as needed to drive learning and positive change. Understand how to support patients in reading medication bottles, following dosage instructions, and refilling prescriptions, while also integrating literacy strategies, such as understanding nutrition labels, making healthy food choices, and addressing food insecurity through tools like healthy food boxes.
