This session will describe a model of care employed by the Boston Health Care for the Homeless Program’s (BHCHP’s) Family Team to provide high-quality, comprehensive medical services to families and youth experiencing homelessness. Using cases, the speaker will describe the key strategies employed by the team, including outreach and engagement, “one-stop shopping,” and community collaboration.
